DEPOSIT

Deposit is $1000 per procedure scheduled

The deposit is due when patient schedules a surgery date OR desires to lock in price quote for one year. If patient locks in price quote, the surgery must be PERFORMED within 365 days that the quote was prepared (date printed on the quote)

The deposit is NOT REFUNDABLE for any reason (medical or non medical) at anytime.

BALANCE

Balance is due at preop visit OR 7 days before surgery (whichever is SOONER)

Balance must be paid by credit card, signed financing agreement, cashiers check, money order or cash. PERSONAL checks cannot be accepted for the final payment.



RESCHEDULING FOR NON MEDICAL REASONS

If the surgery (or part of surgery) is rescheduled for any PERSONAL (non medical reason) within 30 days of the scheduled date, the patient must pay a rescheduling fee equal to the deposit amount.

RESCHEDULING FOR MEDICAL REASONS

If the surgery (or part of the surgery) is rescheduled for any MEDICAL reason at any time (with documented proof of medical problem from primary care physician and documentation approved by surgeon) no rescheduling fee will apply. If surgery is rescheduled for a date more than one year from the date of quote, the patient will be responsible for any change in fees for the procedure, OR, anesthesia or implants.

CANCELLING FOR NON MEDICAL REASONS OR PREGNANCY

If the surgery (or any part of the surgery) is cancelled for ANY REASON, the deposit is ALWAYS NON REFUNDABLE.

If surgery is cancelled prior to 14 days of the surgery date, any amount paid over the deposit amount will be refunded.

If surgery (or any part of the surgery) is cancelled within 14 days of the scheduled date, the FULL SURGEONS FEE is NON refundable.

CANCELLING FOR MEDICAL REASONS

If surgery (or any part of the surgery) is cancelled prior to 14 days of the scheduled date for any MEDICAL REASON FOR WHICH THE SURGERY CANNOT BE RESCHEDULED WITHIN ONE YEAR (with documented proof of medical problem from primary care physician and documentation approved by surgeon) the full amount paid will be REFUNDABLE (including deposit).

If surgery (or any part of the surgery) is cancelled within 14 days of the scheduled date for any MEDICAL REASON FOR WHICH THE SURGERY CANNOT BE RESCHEDULED WITHIN ONE YEAR (with documented proof of medical problem from primary care physician and documentation approved by surgeon) the surgeons fee and implant fee paid will be refunded, however the OR and anesthesia is NOT REFUNDABLE.
